  • Enter your first and last name as they appear on your insurance card

  • Do not use nicknames or preferred names

  • You can bypass the insurance field by selecting “cash”. Your insurance company will be billed as long as we have your current insurance information on record.

  • It may take up to two business days to receive confirmation that your appointment request was accepted.

  • Occasionally an appointment request may be declined due to a scheduling conflict. Should this happen, you may submit a second request online or you can message me through the patient portal to schedule.
